Some of the unique characteristics of the Asian face during an Asian facelift include a flatter facial skeleton with heavier tissues that cause more sag to the lower face, a larger accumulation of fat in the lower face and neck area, more prominent droopy buccal fat pads (which are the cheek fat pads), and thicker skin.
Because of these different characteristics, Asian patients tend to age differently than other ethnicities. Their thicker skin does not show fine wrinkles but Asian patients tend to develop deeper sagging tissue and visibly have prominent neckbands. In order to correct these problems, a facelift that focuses on the deeper musculature and structure of the face is important as more superficial facelifting procedures will not correct these problems.
Reduction of the buccal fat pads in the cheeks and fat in the neck is also an important part of the procedure to contour the cheeks and neckline.
Another important consideration in the Asian Facelift is there is a tendency to develop hypertrophic scarring and keloid scars much more frequently. During facelifting, special measures must be taken to avoid poor scarring.

Understanding the Asian face, Dr. Jacono performs a deep plane facelift, doing Asian plastic surgery in NYC. The deep plane facelift lifts the deeper structure of the face including the drooping muscles of the aging face and neck called the SMAS (superficial muscular aponeurotic system) in the face and the platysma in the neck.
This is important to support the thicker and heavier tissues of the Asian face to create a more rejuvenated appearance and one that will last long term. The deep plane approach will also allow access to the buccal fat pads of the cheeks which are larger in the Asian face so that they can be reduced. Additional Neck fat can also be contoured through the deep plane approach.
With this deeply supported lift under the muscles, there is no tension or pulling on the incision lines once closed. This tension-free closure allows the incision to heal like imperceptible fine lines so that hypertrophic or keloid scarring can be avoided in the Asian facelift.
